How Long Does a DOT Medical Certificate Last?
A DOT medical certificate may be valid for up to 24 months when medically appropriate. The examiner can issue a shorter certification period.
The maximum is not automatic
The common ‘two-year DOT card’ description states an upper limit, not an entitlement. The examiner considers relevant medical factors and any need for closer monitoring when deciding the certification period.
Use your own documented expiration date
Review the date on your certificate and corresponding licensing record. Do not calculate the expiration by assuming everyone receives the maximum duration. If documents conflict, contact the appropriate agency.
Plan for the next evaluation
Schedule before expiry and request specialist or treatment records in advance if relevant. Renewing certification generally requires another qualifying examination and a new examiner decision.
Account for electronic reporting
Since the 2026 NRII transition, state licensing records and electronic medical certification reporting are important for Utah drivers. Paper copies alone should not be assumed sufficient where the current federal rules require electronic reporting.
Official references and next steps
This educational material is not medical advice, not an examiner’s individual qualification decision, and not a guarantee of a medical certificate. Ask a licensed clinician about your medical condition. Federal and Utah guidance may change.
