DOT Physical Medical Requirements

DOT physical qualification is defined by federal medical standards, with individualized assessment by an FMCSA-certified examiner.

What the regulations cover

The interstate physical qualification framework under 49 CFR 391.41 addresses several medical factors affecting safe commercial driving. The examiner reviews history, medications and physical findings; some medical conditions require specialist information, an exemption or follow-up. There is no single universal pass/fail checklist for every condition.

Which drivers must meet these standards

Medical certification obligations are linked to the type of regulated commercial operation, federal and state exceptions, and the driver’s self-certification category. Utah DLD describes four operating categories. Confirm yours rather than relying only on the CDL class printed on a license.

Testing and documentation

The regulated examination includes blood pressure/pulse, vision and hearing assessment, urinalysis, and a physical examination. Relevant records such as treatment updates or specialist notes may be required for individual conditions. A DOT urine check is not automatically a drug test.

Professional judgment and certification

A medical examiner listed in the National Registry evaluates the results under applicable standards and determines if certification is appropriate. A certificate can be issued for up to 24 months in suitable cases, or less when monitoring is necessary.
