DOT Medical Examination Forms and Driver Paperwork
FMCSA specifies medical examination reporting and certificate forms used during the DOT qualification process.
Medical examination report
The examination paperwork collects health history, examination findings and related information needed by an authorized medical examiner. Drivers should provide accurate information and ask the office whether forms are completed in advance.
Medical Examiner's Certificate
When appropriate, the examiner issues the relevant medical certification documentation. It is distinct from a driver’s CDL and from any separate employer paperwork.
Condition-specific documentation
Certain drivers may need additional professional assessment or program-specific documentation, such as forms used in insulin-treated diabetes situations. Do not download random outdated forms without checking the current FMCSA source.
Current federal and Utah reporting
Utah and federal procedures specify how relevant medical information reaches a licensing agency. Changes in electronic reporting make it important to use current official guidance and confirm any recordkeeping questions.
Official references and next steps
