MEDICAL EXAMINER'S CERTIFICATE

DOT Medical Cards: Certification, Validity and Reporting

The term “DOT medical card” commonly refers to the Medical Examiner’s Certificate issued when a driver meets the relevant medical qualification requirements. The examiner, not the website or receptionist, determines whether certification is appropriate. In Utah, CDL medical certification records also involve current electronic reporting procedures, which changed in October 2026.

What the certificate actually means

A Medical Examiner’s Certificate documents the examiner’s medical qualification decision at the time of the DOT physical. It is not a commercial driver’s license, an insurance card or a general assessment of every aspect of a person’s health. It is issued only when the examiner determines the applicable standards are met.

How long certification can last

When appropriate, a certificate can last up to 24 months. An examiner may choose a shorter interval based on medical considerations, or request additional information. Planning for the specific expiration date on your own record is more reliable than assuming all medical cards last two years.

Electronic medical certification in 2026

As of October 12, 2026, FMCSA indicated that paper certificates were no longer acceptable as medical qualification proof for CDL/CLP drivers licensed in states other than Alaska under the National Registry II transition. Drivers should consult current FMCSA and Utah DLD instructions and verify that their licensing record reflects the correct medical certification status. This is distinct from an examiner providing a copy of exam results to the driver.

Missing, incorrect or expired information

If your records are missing or out of date, check current Utah DLD guidance and contact your licensing agency. For a renewal examination, bring your prior certificate and supporting medical information. DOTSTG can explain appointment logistics by phone but cannot change state license records.

Questions about dot medical cards: certification, validity and reporting

For Utah CDL/CLP drivers, current reporting requirements should be confirmed with FMCSA and Utah DLD; paper alone should not be assumed sufficient.
No. Certification requires an individual decision by an appropriately certified medical examiner.
