EXAM CHECKLIST
What to Bring to Your DOT Physical Examination
Before a DOT physical, bring identification, a complete list of medications, any glasses or hearing aids normally used, and medical records relevant to your history. Some conditions require additional documentation. DOTSTG’s St. George office can clarify what records to bring when you call (435) 674-5147.
Identification and current certification
Bring a valid photo ID or driver’s license. If you are renewing medical certification, take the prior Medical Examiner’s Certificate or a record of your expiration date. Double-check the current rules for your CDL operating category separately with Utah DLD.
Medicines, conditions and relevant records
Create a current list of prescription medications, doses and over-the-counter medicines or supplements. Bring clinician notes or specialist information requested for conditions under evaluation. Do not change medication schedules for a DOT exam without advice from your treating clinician.
Corrective lenses and hearing devices
Bring the glasses, contact lenses or hearing aids you normally use. Vision and hearing are part of the applicable examination standards, and the examiner documents findings. Some individuals need additional professional evaluation or supporting records, so ask ahead if you have a known issue.
What to expect during the visit
The examination generally includes a review of your history, a vital-sign check, vision and hearing assessment, urinalysis and a physical examination. DOT physical urinalysis is not equivalent to a separate DOT drug test. The examiner decides certification based on the applicable standards.
